Heizer Middle School Athletics
All student athletes wishing to participate in 8th grade football/athletics this year need to come by Heizer Middle School as soon as possible in the next few days and pick up your physical form and other forms to complete your paperwork. Also, this year Athletes will have to take a concussion course online and turn in the certificate to the office. The office will have the website link to go to. lf you do not have access to a computer, you may use the library at Heizer Middle School to complete the online paperwork. A parent must be present to sign off on the paperwork. lf you need insurance, we have information in the office to help you with this process. Athletes participating in football when you complete your paperwork come by Heizer Middle School and we will assign you a locker and equipment. lf you have any questions concerning football, come by Heizer Middle School and visit.
